Information for New Patients

YOUR FIRST VISIT
After being checked in by our friendly front desk team, you’ll be escorted to your treatment room where you’ll meet Dr. Shwarts. He’ll chat with you for a few minutes so he can get to know you and briefly go over your dental history, and he’ll also ask if you have any pressing concerns about your teeth. He’ll then perform a thorough oral exam, and afterward, he’ll discuss his observations with you and go over your treatment options. Once all of your questions have been answered and a care plan has been agreed upon, you’ll be brought back to the front desk to schedule your next appointment.
NEW PATIENT FORMS
To make your check-in process much easier and help your appointment start faster, you now have the option to fill out our patient forms before you’re sitting in our dental office. Just click on the link below to download them, and be sure to bring the completed forms to our dental office on the day of your visit. You can still fill out the forms in our dental office if you choose, just please allow 15-20 minutes to do so. If you have any issues downloading them, please let us know, and we can email them to you.
